How do we know when health begins to slip into disease? IMMEDIATE aims to answer this question by exploring the “health-to-disease” transition in chronic inflammatory conditions. The project draws on expertise from across Europe to analyse diet-microbiome-metabolite-immune axis. This complex interaction refers to how an individual's diet, gut microbiota, metabolites, and immune system can influence their health.

Through AI-powered predictions, the consortium is building a platform to personalise prevention for conditions such as obesity, diabetes, cardiovascular, kidney and neurodegenerative diseases. The goal: empower people to take more control of their wellbeing and prevent disease long before the first symptoms occur.

Non-communicable diseases (NCDs) rooted in chronic inflammation are rising globally. Obesity, cardiovascular disease, diabetes, kidney dysfunction, and brain-related conditions are claiming more lives and resources each year. But while we know lifestyle and environmental factors play a role, exactly how these factors cause a shift from health to disease remains unknown.

IMMEDIATE sets out to solve this by analysing different systems. By deeply mapping diet, stress, activity, immune markers, and microbiome profiles, the project hopes to predict risk early and empower individuals to take control before symptoms appear. It’s not just about treatment; it’s about truly preventative medicine.

IMMEDIATE combines data from three cohorts and a proof-of-concept study conducted within the project. This includes data about the lifestyle, stress levels, gut microbiota composition, immune system status, and more. By feeding this information into robust AI models, the team creates predictive patterns that show when a person is approaching a disease threshold. These insights will be accessible through a user-friendly platform designed for clinicians, researchers, and patients alike. It's a step toward a future where the first signs of chronic disease can be detected and addressed before they manifest.
